38 years old aged male patient had swelling on left cheek and paresthesia on left lower lip until the molar teeth area have used antibiotics for two weeks by prescribed an general dentist. Root canal treatment have been started for left lower premolar teeth since the complaints didn’t end. Due to severe pain and paresthesia, the patient have referred to us. On x ray film a large cyst whose apice located on left lower premolars and also broken file on the first premolar tooth were detected. Severe percussion was observed on premolars whereas other teeth were normal. On computed tomography the lesion was related with and was pressing on mental foramen.
Canals were cleaned and a temporary filling done without placing any intracanal medicament into canals. Firstly every 2 days then weekly dressing have done. Meanwhile a great amount of drainage have suppplied from first premolar. After 16 days swelling have relieved while paresthesia was proceeding. Calcium hydroxide was placed to second premolar tooth. On 23rd day, decreased drainage was observed on first premolar, calcium hydroxide placed to this tooth. A month later, paresthesia have relieved completeley by the way little drainage was still proceeeding from first premolar. Triple antibiotic paste was placed into canals of both teeth. Monthly dressing kept on. On 5th month appoinment, root canal treatment was finished since the teeth were asymptomatic and the canals were dry.
